- Carefully cut a 1 inch by 1 inch sample of the material
- Enclose the sample in a zip lock bag
- Please also include your name, phone number, and email address with a brief description of the area in the home where sample was retrieved
- Enclose a check for $50 Payable to Your Home Matters
